OBIEE 11g : Scale up of Presentation Server in Enterprise Manager Fails on Activate Changes with 'Commit failed with 0 errors and 2 warnings' (Doc ID 2284041.1)

OBIEE 11g was scaled out horizontally across 2-nodes, it was discovered that the Presentation Server (OBIPS) was no longer deployed / running in instance2 on Node-2

On Node-2, <MW_HOME>/instances/instance2/bin/opmnctl status does not list the "coreapplication_obips1" component; and there is no <ias-component id="coreapplication_obips1"> entry in the <MW_HOME>/instances/instance2/config/OPMN/opmn/opmn.xml file.

In Enterprise Manager > Business Intelligence > coreapplication > Availability > Processes, see there is no "coreapplication_obips1" for instance2.

In Enterprise Manager > Business Intelligence > coreapplication > Capacity Management > Scalability, see Presentation Servers = 0 for instance2.

To scale up OBIPS on Node-2: Lock and Edit Configuration > Raise the above value from 0 to 1 > Apply > Activate Changes ... but see the following error is thrown:-

Activate Changes - Completed Successfully
Commit failed with 0 errors and 2 warnings. Problems reported by: <server>:coreapplication_obips1, <server>:coreapplication credstore.

The following errors are written in the Enterprise Manager log file <BI_DOMAIN>/servers/AdminServer/sysman/log/emoms.log:-

The following were also observed:-
1. <BI_DOMAIN>/config/fmwconfig/biee-domain.xml (On Node-1 and Node-2) shows a "coreapplication_obips1" in instance2
2. <BI_DOMAIN>/opmn/topology.xml (On Node-1) shows a "coreapplication_obips1" in instance2
3. <MW_HOME>/instances/instance2/config/OracleBIPresentationServicesComponent exists but is empty
4. <MW_HOME>/instances/instance2/... contains various other OracleBIPresentationServicesComponent sub-folders
