OBIEE: "No LDAP Connection Available To Process Request For DN: <dn>" Error When Logging In

While trying to login to answers (http://<server>:<port>) users get a "Unable to Sign In" message.
Upon checking the managed server log (bi_server1-diagnostics.log) you see this error message:
