12C Issue With Visual Analyzer "Create Data Source" (Doc ID 2326777.1)

Issue with Visual Analyzer "Create Data Source"

The issue is with Visual Analyzer when trying to create a data connection with two different source:
1) From a File
2) From Oracle Applications

Also issue is specific to one particular environment where other environments works fine

Issue 1/Environment 1: After launching VA and selecting "Create Data Source",  get three options: "From a File", "From Oracle Applications" & "From Database". After selecting "From Oracle Applications", the progress bard (blue in color) seems to go from Left to Right and almost reaches the right side of the window but it seems to hang and does not display a window for entering new connection information.

Issue 2/Environment 1: After launching VA and selecting "Create Data Source", get three options: "From a File", "From Oracle Applications" & "From Database". After selecting "From a File", able to browse for the file but when i select and upload it, it does not upload the file. The error message is: File upload failed. .xlsx is an invalid file. Only Microsoft Office Open XML Workbook Files (.xlsx) are supported.

The following error occurs in managed server log

Request status(500) : error = {"code":50000,"message":"DSS understood the request, but unable to perform the operation due to internal error","subMessage":"Unable to commit the dataset to Directory Service"}sss
