OBIEE 11g : SOAP Invocation Error When Adding Action Links (Doc ID 2417465.1)

Symptoms

When trying to create action from analysis, the following error occurs:

Soap invocation failure. HTTP error code: 'env:SenderUnexpected character '<' (code 60) (expected a name start character) at [row,col {unknown-source}]: [1,487]'.

User is able to click OK to pass this error and then edit/create action links.

However when creating web services(Invoke a Web Service) , the same error again occurs again in the nqserver.log, below error reported:
