BDD: Cannot Reset Password If Captcha Does Not Render Image (Doc ID 2486641.1)

BDD 1.3.0 to 1.4.0

When users try to reset their password by selecting the "Forgot my password" link, it gives them a page to enter their username and below the username field is an image that has a lettering that is always not readable. Captcha is not rendering the Imagae.
This image is suppose to have letters or digits that is expected to be entered in a field below it for verification.

 Related issue happens when the Administrator tries to change the password, in which case no captcha is needed but it fails with:
