Unable to Display Custom Links

Symptoms
You have followed the instructions in the System Administrator's Guide (11g or 12c) and have:
- Created the customlinks.xml file
- Added the CustomLinks element
- Assigned the correct privileges
- Restarted the services
However, once you login to analytics (http://<server>:<port>/analytics) the custom links are not displayed.
