Opatch fails with the following errors when attempting to apply ADF patch 25113405 to Hyperion server :

Finish the Rollback init script for Patch "21240419" at Sat Jan 05 09:56:48 MST 2019
[Jan 5, 2019 9:56:48 AM] Patch 21240419:

Copy Action: Source file "/s03/Oracle/Middleware/oracle_common/.patch_storage/21240419_Jun_17_2015_17_11_29/files/modules/oracle.adf.mo
del_11.1.1/adfm.jar" does not exist.
