OBIEE 12c: FMW Reconfiguration Step Failed with Error "CFGFWK-64254, CFGFWK-60953" during Upgrade from OBIEE 12.2.1.3 to 12.2.1.4

Symptoms
On Oracle Business Intelligence Enterprise Edition (OBIEE) 12.2.1.3.x version,
- Trying to upgrade OBIEE version from 12.2.1.3 to 12.2.1.4
- Pre-upgrade Readiness Check completed without errors.
- Upgraded Schemas using the Upgrade Assistant(UA) successfully.
- During Reconfiguration of BI Domain, getting the below errors in the Reconfiguration Wizard:
ERROR:
CFGFWK-64254: Error occurred in validating domain phase execution"
Encountered Error: CFGFWK-60953: An application or library was not relocated to the new MW Home.
CFGFWK-60953: An error in the reconfiguration templates resulted in the following deployments being left in the original MW home:
opss-rest state-management-provider-memory-rar
DMS Application#12.2.1.1.0
coherence-transaction-rar
Changes
The two installations of OBIEE 12c were done in the following folders -
12.2.1.3.0 : Old FMW installed directory : /./MiddlewareHome/wlserver (for example)
12.2.1.4.0 : New FMW installed directory : /./MiddlewareHome/OracleHome_12.2.1.4/wlserver (for example)
The New FMW directory for BI 12.2.1.4 is still under /./MiddlewareHome (the FMW home of 12.2.1.3.0).
