Hyperion Planning/ Essbase : Error "The form <form name> is invalid" when Opening a Web Form, the Error Started after Planning Refresh to Essbase (Doc ID 2535865.1)

Symptoms

Error "The form <form name> is invalid" when opening most Planning web forms. The error started for most web forms after Planning refresh to Essbase. The form or metadata has not changed for the Planning application   

The "Duplicate member names allowed" is changed to true randomly and running a second refresh changes the outline property back to false. Sometimes several refreshes are required. The "Duplicate member names allowed" property can be checked in Essbase Administration server console by opening the Planning application Essbase database outline. The   "Duplicate member names allowed" property is shown in the Properties tab

The issue has happened with  Essbase version has been 11.1.2.4.023 and higher on Linux 
Hyperion Planning versions are 11.1.2.4.006, 007, 008 
The "invalid form" error happens if the form has members that have shared  members 

NOTE: The Duplicate member names allowed property cannot be changed back to false, i.e. unique member outline,  in Essbase manually 

NOTE: The error "The form <form name> is invalid" can also occur if there is no connection to Essbase when opening Planning forms or if the form is invalid.  

Changes

No changes with the Planning applications.  The form or metadata has not changed for the Planning application
