Symptoms

Hyperion Planning Database Refresh Fails after adding new dimension members. The error on the Planning refresh screen will show the following error for example 

Status: Failed

Step 13 of 39: [Project] Adding members to dimension Entity

<First Entity member name that was added>

The Planning_WebApp.log shows the following errors
