User Specific Planning Login Problems

After Planning login the user is redirected to another Planning application.
After Planning login the HTTP 404 error is displayed.
After a Planning upgrade users receive error "Your session has timed out or was terminated by the application owner or an administrator. Please log on again." when login for first time
A user's Planning login takes forever.
- Issue occurs after migrating an application.
- A user tried to open a large form which caused Planning to become unresponsive.
