Information in this document applies to any platform.
1. An employee is hired and an mailing address is entered.
2. The employee enrolls in benefits through Self Service Open Enrollment and adds a dependent with a different mailing address
3. The employee accesses the Employee Self-Service History Reference P085585 and chooses a Reference Number link for Open Enrollment and chooses the link for Plans and Plan Options History.
4. The mailing address shown under the employee name for the Statement of Benefit Changes is incorrectly the mailing address of the dependent. It should be the mailing address of the employee.
