E1: 17: Changing Issue QTY Causes Unique Constraint F40UI74 Primary Key Violations

When changing the Order Quantity on the Issues Program (P31113)to '0' and
tabbing out of the field then back in and changing it back, the logs will
generate a Duplicate Unique Constraint Error. According to the client is will
also periodically cause the COK (Call Object Kernel) go to a zombie status

OCI0000178 - Unable to execute - INSERT INTO PRODDTA.F40UI74
OCI0000179 - Error - ORA-00001: unique constraint (PRODDTA.F40UI74_PK) violated
JDB9900401 - Failed to execute db request
JDB3400009 - Failed to perform Insert for F40UI74
