Symptoms
When opening "My System options" from user drop-down the application opens but the name shows as:
"My System Options - User Default Revisions"
This issue is observed in all the supported browsers. Similar issues are observed in other forms as well where the HTML entity codes for characters are displayed in form headers instead of the characters they represent.
