E1: AIS: When Running An EnterpriseOne JET Application Based the provided Samples, the Form is not Displaying in the Local Browser from Jdev Tool

Creating an EnterpriseOne JET Application Based on the sample Template downloaded and installed to local Jdeveloper tool.
Using the documentation found here and configuring the AIS Server.
On Step # 3, it says "Change USER_NAME and PASSWORD to be a valid user and password on the HTML Server that is used by the AIS Server."
Where does the USER_NAME and PASSWORD need to be entered in order to test the JET application locally?
Also, when testing, the form is not displaying correctly and there is a browser error related to security.
