When reviewing the F00950W table, there were no records in it. After regenerating the table, the customer was then able to enter a UDO action security record.

Note: Regenerating the table will remove all records and should only be done when the table is empty or when the records are corrupt.  Therefore if there is data in the table, first make a backup before doing anything.
