E1:ORCH Error in Orchestrator When Clicking on the Service Request Menu Item For a Specific User (Doc ID 2428586.1)

Symptoms

This issue may only be replicated if using 9.2.1.7 tools release and Orchestrator 5.x. Have many users that are working fine when using Orchestrator. However, one user is having an issue accessing the Service Request screen. When this user logs into Orchestrator, they click on the Service Request button from the menu and sometimes nothing happens and sometimes they see the below error on the screen:
