E1: ORCH - Cannot Save Orchestration Outputs in Studio Version 6.1.3 on Tools 9.2.2.5 (Doc ID 2477004.1)

Symptoms

In Orchestrator Studio, within an Orchestration, add outputs in the Orchestrations(includes custom Groovy script, Data Request, Form Service Request, etc)  and save option indicates that the record is saved successfully.

However, when going back into to the Orchestration and view the outputs, there is nothing showing and none of the outputs are saved. 

For example,

One Orchestration that calls a simple Data Request Service Request to fetch the Alpha Name and AB Type from the F0101. Click the Orchestration Output button:

Next, click the Add button:

 

 

click the select button for each output item, and push the OK button. Close the outputs form and save the Orchestration.

When reopening the Orchestration, and clicking on the Orchestration Outputs button, there are no more outputs and it looks like this:
