Custom Service Request Tests Successfully But Fails When Added To Orchestration

Customer created a custom service request using Groovy to return the line number for updating a grid.  Using the "Test" button within the service request they are able to get the expected result but once attaching the service request to the orchestration getting the following error:

The original steps for creating/testing the custom service request was to create a form request, run the form request to get the JSON response.  Then use the response JSON as the form data input to the custom service request.  It seems as if the JSON response is different once the form request and customer service request are connected.
