Symptoms

After creating and sharing a personal form for an application, the personal form is not appearing in the list of available forms when defining CAFE1 layout. For example: If a personal form is created for Application P0411-Form W0411K-Version ZJDE0001 and it is shared for public, this form does not appear in the dropdown list of forms when defining a cafe1 layout on any application.

The layout manager's form selector dropdown does not expand to show this form above :
