Symptoms

Correct the instruction text so that the 2020 1095-C template is the same as the IRS form.
IRS form: https://www.irs.gov/pub/irs-pdf/f1095c.pdf

1. Wording "For Privacy Act and Paperwork Reduction Act Notice, see separate instructions. Cat. No. 60705M" was incorrectly at the bottom of page 3, it should have been at the bottom of page 1.

2. Wording "(Continued on page 4)" was missing from page 2.
