Symptoms

 P54HS00/W54HS00B automatically adds media objects.
From P54HS00 click add, then click Save and Continue, add details to the incident, then click Save and Close.
Go to the Investigation section, notice there is an empty text file automatically generated, not added by the user.
The problem is that due to the security rule, now there is an empty media object. There is no way to Change or Delete.

STEPS TO REPRODUCE:
1. Set security (P00950) for a User where Object Name: W54HS00B, View, and Add = Y, Change, and Delete = N.
2. From P54HS00 click add, then click Save and Continue, add details to the incident, then click Save and Close.
3. Open the incident, go to Investigation section, notice there is an empty text file automatically generated, not added by the user.
4. The problem is that due to the security rule, now there is an empty media object. There is no way to Change or Delete.
