E1: PERSFORM: Personal Form Does Not Display Properly When a Group Box is Hidden (P54HS116|W54HS116B)

Symptoms
When a Personal Form is created to hide a Group Box on a form with multiple group boxes such as in Incident User Defined Data Revisions (P54HS116|W54HS116B), the Personal Form does not display properly. Depending on the group box hidden, either the entire form is blank, or the header portion is missing and/or a group box is missing.
If No Personalization is selected in the form, and the group boxes expanded prior to selecting the Personal Form, the personalized form displays properly. However, when the Personal Form is set as default, or selected from the drop-down, the Group Boxes are collapsed by default and the Personal Form does not display properly.
Steps:
1. Fastpath to P54HS00, Manage Incidents, add an incident or select an existing incident (e.g. Incident Number 389).
2. In P54HS00, Manage Incidents, select the Incident Number from Step 1 (Edit Incident), note the Record Type used and the Incident Number.
3. Fastpath to P54HS115, Work with Incident User Defined Data Templates, add an Incident in User Defined Data Template Revisions. Populate Record Type (matches incident), Data Group (e.g. SAFETY), Field Number 1. Data Description is free-form (e.g. Date of Observation). Field Type is Date.
4. In P54HS115, Work with Incident User Defined Data Templates, add another Incident in User Defined Data Template Revisions. Populate Record Type (matches incident), Data Group (e.g. SAFETY), Field Number 21. Data Description is free-form (e.g. Describe Observation). Field Type auto-populates.
5. Fastpath to P54HS116, Work with Incident User Defined Data, add an Incident in User Defined Data Revisions, Form W54HS116B. Enter the Incident Number from step 1 and Data Group. Populate field 1 and 21. Save. Close. Click Find to refresh the grid.
6. In P54HS116, Work with Incident User Defined Data, select the Incident Number. In Form W54HS116B, Incident User Defined Data Revisions, click on the Personal Form icon to create the Personal Form.
7. In Personal Form Manager, right click and hide Group Box Fields 11-20.
8. Save and close the Personal Form Manager.
9. In P54HS116|W54HS116B, Incident User Defined Data Revisions with the Personal Form selected, the form does not display properly. The form header is missing and only the last group box displays.
No Personalization Selected Test:
1. In P54HS116|W54HS116B, Incident User Defined Data Revisions, choose No Personalization from drop-down.
2. In P54HS116|W54HS116B, Incident User Defined Data Revisions, expand all Group Boxes. Select the Personal Form from the drop-down and the personalized form displays properly.
