E1: OMW: Unable to Add, Check-in or Promote Versions in OMW Error "Cannot Fetch Release Level for Data Source Versions" (Doc ID 639392.1)

Symptoms

This error can show itself in different scenarios.  Below are several examples of the way a user may experience this issue.

Issue 1:

When trying to check-in a new batch version in Object Management Workbench (OMW), getting error message "general error in method check-in" and the version stays checked out.  The following error messages appear in the jdedebug.log

Issue 2:

When attempting to promote a project with a single application or batch version, the OMW logging shows that the transfer failed and was rolled back.  OMW status bar shows the error "General Error in method, Status Change". The jdedebug.log file shows the error "Cannot fetch Release level for Data Source Versions - XXXXX. This must be entered in application P00948".  Why is this project not promoting and how can I get it to promote successfully?

Issue 3: 

When attempting to add a new batch or interactive version in some environments, no errors are given, however, no version record is created.  Why not?

Issue 4:

Unable to save to the save location with an error for batch version of "General Error in Method" and for interactive versions "RDB Error".
