E1: 90CB: Converting Lead to Opportunity Causes a "Not Found In User Defined Code Error" (018A) (Doc ID 651450.1)

Symptoms

Trying to convert a lead to an opportunity and this results in "Not Found In User Defined Code Error" (018A). From the lead program (P90CB010) you can convert a lead to an opportunity. Under the classification screen we cleared all required fields we could see, which includes category code 1. Tried filling category code 1 with either the default value or another value and the error is the same both times.
