Symptoms

Every time our users enter a Work Order (P48013) via the web client, the same Media Object text attachment comes predefined with text.  Every work order has the same attachment.  I believe this attachment already existed on another work order, but it seems to have somehow become the default attachment for all work orders. This only happens on the Web (HTML) client.  Why does this occur and how do we remove this default attachment?
