E1: SVM: Server Showing Up Twice Under Managed Home Location In Server Manager Console (Doc ID 838307.1)

On Server Manager Console, it shows that the Managed Home Location (<SERVERNAME.DOMAINNAME1>) is up and all its Managed Instances are up & running. But when clicked on <SERVERNAME.DOMAINNAME1> and it shows that all JVM managed instances are down. When clicked on either of JVMs and it says that the JVM instance is down. But on the left side, it shows the JVM count as 2 and shows the user count also. Runtime metrics also visible. But the configuration section & right hand side related to that JVM is not visible.  The blank page is displayed with following 2 messages:

Warning Message 1:

On the SMConsole homepage, there is one more duplicate entry for appsrv1 under Managed Home Location section with a different name as "<SERVERNAME.DOMAINNAME2>".
But the actual server name is : "<SERVERNAME.DOMAINNAME1>". 

The other entry is an invalid/fake one which is creating conflict in the JVM instances that were installed on that web server, due to which the JVM instance details are not shown. The JVM instances under the invalid entry are not even accessible.
