E1: SEC: Security on Role Not Working Due to Duplicate Role Sequence Number in the F00926 Table (Doc ID 859804.1)

Symptom 1

I have applied Row Security at the role level.  When users signs-on with that role assigned, the row security is not being applied.  I have verified that the appropriate role with the security defined is assigned to the users, the row security for the Data Dictionary item has been enabled and a full Package Build has been built and deployed.  Why is the row security not being applied correctly?   

Symptom 2

During login screen on a development fat client, user get invalid role error when try to choose role or choose environment. After login, the user don't get fast path.
