BPEL Process Hangs, Full Table Scan on WORK_ITEM Table And No Errors in the Logs (Doc ID 1059100.1)

Symptoms

 In a 10.1.3.4 clustered environment you're seeing a hanging process in which no new instances can be created.  There are no errors in the log files.

Generally, if startup is taking lot of time and a threaddump shows that the startup is blocked on the SchedulerAgent.recover() method when you do not have startup recovery enabled in your environment, you have hit upon this same issue.
