FOP PDF Printing Fails with 500 Internal Server Error in APEX (Doc ID 1064107.1)

Configured FOP to print the report in PDF format. Run the report and click the Print link fails with the following errors:

Apex application page may have this error:

ORA-20001: The printing engine could not be reached because either the URL specified is incorrect or a proxy URL needs to be specified.

The content of the PDF file has the following:

500 Internal Server Error
Servlet error: An exception occurred. The current application deployment descriptors do not allow for including it in this response. Please consult the application log for details.

The following error occurs while opening the PDF file:

Adobe reader could not open <pdf filename> because it is either not a supported file type or because the file has been damaged
To create an Adobe PDF document, go to the source application. Then print the document to Adobe PDF.

Report Printing
--Printer Server - Standard chosen
--Print Server Protocol - HTTP
--Print Server Host Address: <Host Name>
--Printer Server port: <OC4J port>
--Printer Server Script: /fop/apex_fop.jsp
