Unknown Placeholder Exception Occurs in Production Environment Even Though EAR Consists of Placeholders (Doc ID 1065183.1)

Symptoms

After creating some new placeholders in Development environment and redeploying the new EAR (containing the new placeholders) to production, below exception is thrown and new placeholders are not accessible:

####<TIMESTAMP> <Error> <Placeholder> <HOST> <ManagedServer> <[ACTIVE] ExecuteThread: '11' for queue: 'weblogic.kernel.Default (self-tuning)'> <> <> <> <1263286693660> <BEA-402001> <Content Error (Placeholder: /placeholders/placeholder.pla). Unknown placeholder '/placeholders/placeholder.pla'>

Additionally, the following propagation errors can be seen in logfile, stating that a resource was not correctly exported:

ERROR (<TIMESTAMP>): Validating that manual elections have been fulfilled on the destination system...FAILURE
ERROR (<TIMESTAMP>): The following resources are missing from the destination application and need to be added. Propagation does not handle adding these types of resources. They will need to be added manually.
ERROR (<TIMESTAMP>): Resource [Application:PersonalizationService:ContentServices:WLPRepository:ContentTypes:TYPE], Manual Explanation [This resource is required by [Application:PersonalizationService:placeholderService:Campaign] and needs to be added, however it was not found in the inventory file. This is typically because the resource was excluded by scoping. Either add the resource to the destination system manually or ensure that the resource is in scope.].
