Provisioning User to Exchange 2007 is Failing with Create User or Create Mailbox Failed (Doc ID 1076136.1)

Symptoms

Attempting to provision user to Exchange 2007 is failing.
Exchange resource is showing Provisioning.

Clicking on Exchange Resource shows that it is hung on Create Mailbox task.
Clicking on Create Mailbox returned one of the following errors:
- "Exchange invalid log on name provided."
- "Connection refused to host <IP Address> nest exception is:
java.net.ConnectException: Connect" does not correspond to a know Resource Code. Using "Unknown'
- "ExternalEmailAddress is invalid"


Provisioning to AD works fine.


Log shows following error:
