BPEL Process Invoked By A File Adapter Generates More Instances Than Expected

Symptoms
The problem is when monitoring a process that uses a File Adapter to invoke a process, under the Administration tab you observe a number of invoke thread statistics and you expect that to be a direct relationship to the number of instances.
You may see more Invoke Thread Requests than the total number of files that should have invoked the process. The problem is there are instances created for files that have already been processed. This is extremely random and appears more prevalent on slower environments.
