Creating Portal User By API Gives User-Defined Exception ORA-06512 (Doc ID 1083665.1)

Symptoms

In a 10.1.4.2.0, running custom PL/SQL code to create Portal users, there are intermittent problems creating portal users by calling wwsec_api.add_portal_user function.
The function call returns immediately with OTHER exception.

Exception handling ends up in "when others" and this is an example of logging output:

Many user are created as expected, but most calls to add_portal_user seem to fail.
