AXF Installer Fails to Complete and Throws Into the install.out, the Error "Unable to get a deployment manager." (Doc ID 1100907.1)

Symptoms

You execute the AXF install script and it creates the AXF middle-tier database tables, inserts data in the tables, but then fails with the installer.out errors:

axf-createDS:
[oracle:createJDBCConnectionPool] Unable to get a deployment manager.
[oracle:createJDBCConnectionPool]
[oracle:createJDBCConnectionPool] This is typically the result of an invalid deployer URI format being supplied, the target server not being in a started state or incorrect authentication details being supplied.
