How to Fix "Request Header is Too Large" Exceptions in GlassFish Server 2.x (Doc ID 1219253.1)

When the HTTP request headers are bigger than the default size of 4096 bytes, the following exception is thrown in the server.log file of GlassFish Server 2.x
