Attached Content On Managed Attachments Not Visible (Doc ID 1266401.1)

Symptoms

After attaching content to a record using Managed Attachments  and doing a refresh the newly attched content does not show on the Managed Attachments window. This has created an issue where we can only attach content but can not view, edit or even detach content using the EBS/UCM adapter.
