ESB Startup Fails/ESB Service Registration (Deployment) Fails - Error: org.apache.slide.store.impl.rdbms.StandardRDBMSAdapter - ERROR - Failed to insert permission (Doc ID 1266574.1)

Symptoms

You have setup either an ESB single node system or a cluster to use a database based slide repository by following this document:

3.24 Configuring the Slide Repository to use the Database as the Repository

You notice that some or all registrations of ESB services fail.  The following messages are seen in the container logs during startup in $ORACLE_HOME/opmn/logs/<container name>.log:

10/11/09 11:05:47 09 Nov 2010 11:05:47 - org.apache.slide.store.impl.rdbms.StandardRDBMSAdapter - ERROR - Failed to insert permission (/,all,/actions/write-acl)
10/11/09 11:05:47 09 Nov 2010 11:05:47 - org.apache.slide.store.impl.rdbms.StandardRDBMSAdapter - ERROR - Failed to insert permission (/,all,/actions/unlock)

You may also notice that during startup of the containers that the following two messages are NOT seen in the log.xml file found in $ORACLE_HOME/j2ee/<ESB Runtime and ESB Designtime container names, e.g. oc4j_soa or oc4j_esbdt>/log/*_*_*_group_*/oc4j

For the ESB Runtime application/container, this message does not exist:

ESB bootstrap: Runtime Initialized

For the ESB Designtime application/container, this message does not exist:

ESB bootstrap: Repository initialized
