Using SSO and a Virtual Host, Forms 11g Fails With FRM-92550 and FRM-92052 After Logging Fine in SSO (Doc ID 1276865.1)

Symptoms

Using a virtual host, trying to start any form with ssoMode=true fails after logging fine into SSO with FRM-92550 and FRM-92502:

FRM-92550: Negative content-length on a response to a GET to URL
http://myvhost:7799/forms/frmservlet?config=webutil&ifsessid=formsapp.16&acceptLanguage=en-US,nl-BE;q=0.5&ifcmd=startsession&iflocale=en-US
FRM-92052: fatal error: cannot connect to the server at URL /forms/lservlet
