"The Per Server Connection Limit Has Been Exceeded" error (Doc ID 1286183.1)

Goal

We are getting "The per server connection limit has been exceeded" error. We would like how it counts to the limit defined for the engine.How does the engine count to the current limit. Is this a queue? If workspace hits engine 8 times, does it count as one hit or eight? If we use PAPI, does each PAPI hit count as one?
