Cannot Download Files From an APEX Application at Runtime (Doc ID 1299096.1)

Symptoms

An APEX application is being accessed via Oracle HTTP Server (OHS) using the SSL (https) protocol. In addition, the APEX application is using Single Sign-On (SSO) as its authentication method. 

Within the APEX application there is a 'download' link to download a file. On clicking on this link the following error is shown.
