WLS_REPORTS Managed Server Unexpectedly Going Down

Symptoms
WLS_REPORTS managed server is unexpectedly going down causing Reports Servlet to be unavailable. If using in-process Reports Server then Reports server will be also unavailable.
Checking DOMAIN_HOME/servers/WLS_REPORTS/logs/WLS_REPORTS.out shows the following information.
** It is important to review also previous WLS_REPORTS.out files as this files are recycled by default so you will see files called like WLS_REPORTS.out00019 as an example
