10.1.3.5 Patch 8626084 erroring with OiilQueryException (Doc ID 1304940.1)

Symptoms

While applying 10.1.3.5 patch , runInstaller reports
"The installer failed to modify server.xml. Unless this file is modified OC4J will not work correctly. The script /pfna6o/ias/product/asmt_10131/config/oc4j_build.pl must be run without errors for the install to succeed.
"


oraInventory installActions logfile reports the following
