DS 5.X ldapsearch and ldapmodify failed with operation errors / "Ldap - Error Database Index Operation Failed Bad 1050, Err=-30989 Db_runrecovery" (Doc ID 1327147.1)

Operations error when running ldapsearch and ldapmodify against DS 5.X. Now all apps that require Directory server authentication are failing in their Production environment.

# ./ldapsearch -b o=REALM -D "cn=DS_ADMIN" -j ******* uid=userOne-j

ldap_search: Operations error

Also: ldapmodify command, the answer is the following:

ldap_modify: Operations error

Because of this behavior all application experiencing failures in authentication against directory services in productive environment.

Using ldapsearch or ldapmodify and bind as DS_ADMIN  does not work either.
