NPE During Dispatch of the Response of a Request / Response Web Service

Symptoms
When invoking a composite containing a request / response web service from the EM Test Console the response fails to return and the following error is seen in the server log:
Note: The actual errors differ between 11.1.1.3 and 11.1.1.4. Below both errors are listed and labeled.
