Action Menu 'Rma.SearchForm.dID' Java Script Error When Select Action Menu Item 'Metadata History' for folio (Doc ID 1345481.1)

Symptoms

When viewing a folio's metadata history a 'Rma.SearchForm.dID' javascript error is seen.

Here are the steps to produce the error:
1. Create simple folio with a contribution folder assigned
2. Check in the folio
3. Browse to the contribution folder: browser content - contribution folders
<to that folder>
4. View metadata history - js error is thrown. Others (recent reviews and
life cycle also don't work and throw js error).
5. If you go to info page, and THEN select life cycle and metadata history
works without error.
