OIDDAS Fails to Load Correctly After CPU Patch Applied

Symptoms
- After CPUJul2011 was applied, the OIDDAS application fails to load correctly and the image buttons (like GO) are not working.
- Clicking on 'My Profile' tab, returns:

Other possible symptoms, when running http://<host>/oiddas or http://<host>/oiddas/ui/oracle/ldap/das/mypage/ViewMyPage some of the buttons do not show up.
The application.log for OC4J_SECURITY shows:

12/01/11 00:57:48 ui: StoppedPlease note that symptoms and logs are not limited to the ones described above, and in general, any oiddas buttons/tabs functionality or problems after applying a cpu patch is likely to be this same cause.
