OIDDAS Fails to Load Correctly After CPU Patch Applied (Doc ID 1352078.1)

After CPUJul2011 was applied, the OIDDAS application fails to load correctly and the image buttons (like GO) are not working.
Clicking on 'My Profile' tab, returns:
Other possible symptoms, when running http://<host>/oiddas or http://<host>/oiddas/ui/oracle/ldap/das/mypage/ViewMyPage some of the buttons do not show up.

The application.log for OC4J_SECURITY shows:

Please note that symptoms and logs are not limited to the ones described above, and in general, any oiddas buttons/tabs functionality or problems after applying a cpu patch is likely to be this same cause.
