Symptoms

Using Content Server 11.1.1.3 and higher.

The page for editing the JpsUserProvider does not show the fields for editing "Role Prefix" and "Attribute Map".
In fact, all fields after "Default Network Roles" are missing.

Steps to reproduce:

Login to UCM as admin user
Go to Administration -> Providers
Click on the "info" link for the JpsUserProvider
Click Edit button
The Role Prefix and Attribute Map fields are missing
