The IPM Document Index Service Will Not Start And ODBC Error 18456 Is Reported (Doc ID 1373410.1)

Symptoms

The Document Index Server will not start. In the Document Index Server logs, the following error can be found:

ODBC Error: Error 18456, State 28000, Login failed for user 'username'

Changes

This error will be thrown if the username or password for the ODBC source has been changed and DocIndexServer has not been changed to match. 
Or
In Services Configuration, under the Document Index Server tab, an incorrect username or password for the ODBC has been entered.
