SSXA Error Pages Defined in web.xml is Not Displaying Placeholder Defined Content (Doc ID 1374399.1)

Symptoms

The Placeholder Definition (PHD) templates and sub-template code on an Error Handler page do not execute. The following error is reported.

Please find below debug output of the error. There are several of these generated per placeholder.
